Procedures to Request for Disclosure, etc.

1. Method of Request

A person who needs notice of the purposes of use of, disclosure (including disclosure of records of third party provision) of, correction (or addition or deletion) of, or suspension of use (or erasure or suspension of provision to third parties) of retained personal data under the Privacy Act must make a request to the contact point below. The Company will send a prescribed request form in response to the request and the requesting person must send back the form to the contact point designated by the Company after filling in required items. The following documents must be sent with the form for verification of identity of the requesting person and for identification of retained personal data or records of third party provision related to the requesting person. Please note that the requesting person must bear the expenses for sending the request form, etc.

  • (1) Request by principal
    • Original copy of seal registration certificate (that was issued within three months and states his or her current address)
    • Copy of driving permit, health insurance certificate, passport, or any other document issued by a public organization
  • (2) Request by agent
    • Identity verification documents stated in (1) above
    • Original copy of the agent’s seal registration certificate (that was issued within three months and states his or her current address)
    • Copy of the agent’s driving permit, health insurance certificate, passport, or any other document issued by a public organization
    • In the case of a statutory agent, a document proving the authority of statutory representation (family register, written ruling of commencement of guardianship, or the like)
    • In the case of an agent by delegation, letter of attorney

2. Fees

A person who requests for notice of the purposes of use of or disclosure of retained personal data must bear 1,000 yen (including consumption tax) as a fee for the request.
If it is obvious that the expense of disclosure including processing of data subject to disclosure exceeds this amount, the requesting person must additionally bear the amount equivalent to the actual expense for his or her request as a fee for the disclosure. The estimated amount of this fee will be separately notified in advance.
The fees must be paid by wire transfer to the account designated by the Company. Please note that the transfer fee must be borne by the requesting person.

3. Method of response

The Company will make a response to the relevant principal by delivery of a document, provision of electronic records, or any other means prescribed by the Company (including by having the principal come to the Company and playing back a video or voice) after confirming, investigating, and otherwise checking the details of his or her Request for Disclosure, etc. and confirming payment of fees, if any, for the request. The Company will make a response to the principal even in the case of a request by his or her agent except the case of a request by his or her statutory agent.

If the Company cannot make a response to a Request for Disclosure, etc. in the case that it causes a possibility of harming a principal or third party's life, body, fortune, or other rights and interests, a possibility of interfering seriously with the Company implementing its business properly, a violation of a law or regulation, or the like, the Company will notify the reason.
